Position Summary

Completes home studies to determine families' suitability to meet the needs of children in out-of-home care.  Conducts rigorous background checks, home inspections and psycho-social assessments; Orients and trains prospective foster families to the application, home study and placement process.  Responsibilities could include making placement matches and coordinating the placement process.  Monitor up to 30 licensed foster homes or enrolled families;  Complete investigations to ensure foster families' compliance with licensing rules, agency and state policies, and state law.

Provide intensive home-based family intervention, life skills education, and supportive services in order to preserve families, reduce the need for out of home placement, expedite the return of children to their families, and prevent the recurrence of abuse and neglect.  Carries a caseload of two (2) families.  Responsible for: regular visits with the family in their home or community for a period of 28 days, and not more than 42 days; performing required assessments; writing and submitting service plans, safety plans and progress reports; providing concrete assistance and referrals based on the individual needs of each family.  Works a flexible schedule (including non-traditional hours) based on families' needs.

Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their families to resolve abuse and neglect difficulties and to achieve permanency for youth; Coordinate youth's placement within foster care, school and community, DHS and family court.  Manage a caseload of approximately 15 children; Testify in court; Provide documentation for all activities completed.
